When explaining the cause of disc problems, it is common to point to inflammation caused by disc damage or the material that escapes from a torn or herniated disc. However, inflammation and disc herniation are not necessarily the original cause of the problem. They can be the result of an underlying mechanical problem.
This raises an important question:
Why does a healthy disc become damaged or torn in the first place?
A normal adult spine consists of 24 vertebrae—7 cervical vertebrae in the neck, 12 thoracic vertebrae in the mid-back, and 5 lumbar vertebrae in the lower back. There are also 23 intervertebral discs located between the vertebrae.
The spine serves as one of the body’s primary structural supports. It helps protect the spinal cord and nerves while allowing the body to move.
Where two vertebrae meet, they form joints that allow movement between the bones. The spaces between the vertebrae also provide pathways through which nerves travel. The intervertebral discs are often described simply as shock absorbers, but they also play an important structural role. They help maintain appropriate spacing between the vertebrae, allowing the spine to move while helping maintain the space necessary for normal nerve function.
Physical trauma from an accident, collision, or fall can place significant stress on the structures of the spine. When the vertebrae and surrounding structures are subjected to abnormal forces, the ligaments supporting the spine can become overstretched or injured. Changes in spinal mechanics can also increase stress on the intervertebral discs. When discs, ligaments, or other surrounding tissues are injured, the body responds by initiating an inflammatory healing process. Depending on the severity of the tissue injury and inflammation, pain may develop.
Pain often serves as an important warning signal, prompting a person to seek medical attention. However, there is another important concern:
Spinal and disc problems can sometimes progress without significant pain.
Another source of stress on the discs and spinal structures can come from our everyday habits—particularly prolonged poor posture. Ideally, the head should be positioned approximately over the shoulders. This allows the weight of the head to be distributed more efficiently and can reduce unnecessary stress on the neck and the rest of the spine. When we spend long periods leaning forward or to one side, the muscles and supporting structures of the spine must work harder to maintain balance. Over time, these repeated stresses can contribute to changes in spinal mechanics and muscle function. Because these changes often develop gradually, there may be little or no pain in the beginning. A person may simply notice occasional stiffness in the neck, shoulders, or lower back.
As these postural and mechanical changes continue over a long period of time, even relatively minor physical stress may cause the muscles to tighten quickly, making movement uncomfortable or difficult. The problem is that many people do not realize that their everyday posture may be contributing to changes in the way their spine functions. As a result, they may focus only on taking medication to control discomfort rather than identifying and addressing the underlying mechanical factors.
Problems involving the spine and joints can affect more than just pain and muscle tightness. Chronic musculoskeletal problems can influence how comfortably and efficiently a person moves and performs everyday activities. When muscles remain chronically tense in an effort to support or stabilize an area of the spine, the body may expend additional energy maintaining that tension. Some people may consequently experience increased fatigue or reduced physical performance.
However, it is important to understand that spinal problems should not automatically be considered the direct cause of systemic diseases such as allergies, shingles, hypertension, thyroid disorders, stroke, or dementia. These conditions have complex and multiple causes and require appropriate medical evaluation and treatment. What is important is to recognize that overall musculoskeletal health is an important component of quality of life and physical function. Ignoring chronic postural or spinal problems may allow mechanical dysfunction to persist and can make recovery more difficult.
When treating a disc problem, simply reducing inflammation or pain may not address every factor contributing to the condition. If abnormal spinal mechanics are placing excessive stress on a disc, it may be important to evaluate why that stress is occurring in the first place. When the vertebrae are properly supported by strong, well-functioning muscles and the spine is able to maintain healthy movement and alignment, the discs may be subjected to less abnormal mechanical stress. For this reason, a comprehensive approach to spinal care may include evaluating spinal mechanics, posture, muscle function, and the way the body moves as a whole—not simply treating the location where pain is felt.
The goal should not be only to make the pain disappear. The goal is to understand why the problem developed and to address the factors that may be contributing to it. By identifying and correcting contributing mechanical problems, reducing unnecessary stress on the affected structures, and supporting healthy movement and muscle function, the body may have a better environment in which to recover and function properly.
