There are many possible causes of finger and wrist pain, so each case needs to be evaluated individually. However, one of the most common contributing factors is repetitive or improper use of the muscles during everyday activities.
The muscles that control the fingers and wrist include muscles responsible for flexing the fingers and extending the fingers. The finger flexor muscles are primarily located along the palm side of the forearm. They originate around the elbow and forearm and connect through tendons to the fingers. The finger extensor muscles are primarily located along the back of the forearm and also connect through tendons to the fingers. The problem is that many activities we perform every day require us to repeatedly grip objects with our fingers flexed. We also spend long periods typing on a keyboard or using a mouse while keeping our fingers and wrist in relatively fixed positions. When these habits continue for many years, the muscles used more frequently may remain relatively strong while the opposing muscles receive less use and may become weaker. Healthy joint function depends on a proper balance between the muscles that bend and straighten a joint. When one group of muscles becomes significantly weaker than its opposing group, the stronger muscles may place greater stress on the joint and its surrounding tissues. Over time, this imbalance can contribute to abnormal joint mechanics and increased stress on tendons, ligaments, and other supporting structures. This is one reason we may see older adults whose fingers have gradually become stiff, bent, or deformed after years of repetitive hand use. Degenerative changes in the finger joints can also cause the joints to become enlarged, thickened, or uneven in appearance.
Many joint problems do not begin with a single accident or traumatic event. Instead, they can develop gradually through years of repetitive stress and everyday habits. Because these changes often happen slowly, people may not pay much attention to the condition of their joints until pain develops or visible structural changes have already occurred. At that point, restoring normal joint function can be much more difficult and may take considerably longer. For this reason, if you begin to notice even a mild change in the way your fingers, wrist, or other joints feel or move, it is better to have the problem evaluated before it becomes more serious. Early attention and proper joint care may help maintain healthy joint function for many years.
There is also a common misconception about finger exercises. Many people exercise their hands by repeatedly squeezing a soft ball or grip-strengthening device. While this type of exercise can strengthen the finger flexor muscles, it may not address weakness in the opposing finger extensor muscles. If the goal is to improve muscular balance, exercises that strengthen the finger extensors may also be important. One simple exercise is to place a rubber band around all five fingers and repeatedly open the fingers against the resistance of the band. This activates the muscles responsible for extending the fingers. The resistance should be appropriate for the individual’s condition. A thinner or thicker rubber band can be selected depending on the strength and condition of the hand. However, if opening the fingers against resistance causes pain, you should not simply push through the pain. Pain may indicate irritation or injury involving the tendons, ligaments, or other tissues, and inappropriate exercise could potentially make the problem worse. In that situation, a gentler approach may be more appropriate. For example, using a very light weight of approximately 1–2 pounds and performing controlled wrist-extension movements may provide a less demanding way to strengthen the muscles. If a mild, persistent ache remains, gentle warmth may help relax tight muscles and temporarily improve comfort. If acute pain suddenly becomes severe, a short period of ice application may help calm the symptoms.
The condition of the hand does not always exist independently from the rest of the upper extremity. Depending on muscle imbalance and movement patterns, the bones and joints of the forearm or wrist may also become mechanically stressed. In addition, problems involving the neck, shoulder, or elbow may contribute to or aggravate symptoms that are felt in the wrist or fingers.
For this reason, rather than treating only the location where you feel pain, it is important to consider the entire kinetic chain—from the neck and shoulder to the elbow, wrist, and fingers. A professional evaluation can help determine whether the symptoms are coming primarily from the hand itself or whether other areas may be contributing to the problem.
When joint pain develops, many people immediately look for ways to reduce inflammation and discomfort. Medications or injections may sometimes provide temporary symptom relief, but reducing pain alone does not necessarily address the underlying mechanical factors contributing to the problem. If the contributing cause is not identified and addressed, symptoms may continue to return.
This is why it is important for patients to understand their condition and make informed decisions about their care. Don’t wait until a joint becomes severely painful or visibly deformed before paying attention to it. Your joints are designed to support you throughout your entire life. Maintaining healthy movement, balanced muscle function, good posture, and proper body mechanics can play an important role in protecting your joints and preserving your quality of life.
