At NoVA Gonstead Chiropractic, we believe you should understand your treatment options and financial responsibility before beginning care. Our office works with patients using insurance benefits, out-of-network benefits, auto accident coverage, and self-pay options.

Because insurance benefits vary by plan, we recommend confirming your chiropractic benefits and coverage with your insurance company before beginning treatment.

Insurance

We accept many major insurance plans and can help you understand how your benefits may apply to chiropractic care.

Insurance coverage may vary depending on:

• Your specific insurance plan
• Whether our office is in-network or out-of-network
• Your deductible
• Copayment or coinsurance
• Visit limitations
• Chiropractic coverage requirements
• Whether authorization or a referral is required

Even when chiropractic care is covered by your insurance, you may still have a deductible, copayment, coinsurance, or other patient responsibility.

Insurance coverage does not guarantee payment. Your insurance company determines your actual benefits and payment according to the terms of your individual plan.

Out-of-Network Insurance

If our office is considered out-of-network with your insurance plan, you may still have out-of-network chiropractic benefits.

When applicable, we can provide the documentation needed for you to submit claims to your insurance company. Your insurance carrier will determine the amount they reimburse based on your specific policy.

Before starting care, we encourage you to ask your insurance company:

• Do I have chiropractic benefits?
• Does my plan cover out-of-network chiropractic care?
• What is my deductible?
• What is my copayment or coinsurance?
• Is there a limit on the number of chiropractic visits?
• Does my plan require a referral or prior authorization?
• What portion of the allowed amount is my responsibility?

Medicare

We welcome Medicare patients and can explain our payment policies before treatment begins.

Please be aware that Medicare coverage does not necessarily apply to every service provided in a chiropractic office. Medicare has specific rules regarding covered chiropractic services and does not cover all examinations, X-rays, or other services.

Patients may have financial responsibility for services that are not covered by Medicare.

We recommend discussing your specific Medicare coverage and financial responsibility with our office before receiving services.

Auto Accident & Personal Injury Cases

If you were injured in an automobile accident, our office may be able to work with your auto insurance or personal injury case depending on the circumstances of your claim.

We can discuss available payment arrangements and documentation requirements during your initial visit.

Please bring any information you have regarding the accident, including:

• Auto insurance information
• Your attorney's contact information, if applicable
• Accident claim information
• Health insurance information
• Other relevant paperwork

Because every accident case is different, financial responsibility and payment arrangements will be discussed individually.

Cash & Self-Pay Patients

You do not have to have insurance to receive chiropractic care.

For patients who choose to pay directly, we offer self-pay options. Our team will explain the fees for your examination, X-rays when clinically indicated, chiropractic care, and other services before treatment begins.

Self-pay can be a straightforward option for patients who:

• Do not have chiropractic insurance coverage
• Have a high deductible
• Have limited chiropractic benefits
• Prefer not to use insurance
• Have out-of-network benefits
• Want to understand their costs in advance

Payment Methods

For your convenience, we accept several forms of payment, including:

• Major credit and debit cards
• Cash
• Other approved payment methods

Payment is generally expected at the time of service unless other arrangements have been made in advance.

Understanding Your Financial Responsibility

We are happy to help you understand our office policies and provide an estimate of your expected responsibility when possible. However, an estimate is not a guarantee of insurance payment.

Your insurance company ultimately determines coverage and reimbursement according to your individual policy.

If your insurance company does not pay for a service, or pays less than expected, the remaining balance may be the patient's responsibility.

We encourage you to ask questions before beginning care. Our goal is to make the financial side of your chiropractic care as clear and predictable as possible.
