Save Time Before Your Appointment

To help make your first visit as smooth and efficient as possible, we ask new patients to complete the appropriate patient forms before arriving at NoVA Gonstead Chiropractic.

These forms provide us with important information about your health history, current symptoms, previous injuries, and other factors that may be relevant to your chiropractic examination.

What Information Will I Need?

When completing your forms, you may be asked to provide:

• Your personal and contact information
• Current symptoms and areas of concern
• When your symptoms began
• Previous injuries or accidents
• Previous chiropractic or other healthcare treatment
• Relevant medical history
• Current medications
• Insurance information, if applicable

Please provide as much accurate information as possible. Your answers help Dr. Kang better understand your history and prepare for your examination.

Before Your First Visit

Please arrive a few minutes early if you have not completed your forms in advance.

During your first visit, Dr. Kang will review your health history and discuss your current concerns with you. A detailed chiropractic examination will then be performed to evaluate your spine, posture, and areas related to your symptoms.

Depending on your condition and the findings of your examination, X-rays may be recommended when clinically appropriate.

After the examination and review of the findings, Dr. Kang will explain the results and discuss your recommended care plan. In most cases, the Report of Findings is provided at your following visit, allowing adequate time to carefully review your examination and X-ray findings.

If you are experiencing significant acute pain and an adjustment is appropriate, your Report of Findings may be provided on the same day.
